Why Did My eGFR Drop Suddenly? Temporary and Chronic Causes

2 min read

A sudden drop in eGFR can be alarming, but it does not always mean permanent kidney damage. eGFR is calculated from serum creatinine, and creatinine can change with hydration, illness, exercise, diet, and medication. The size and persistence of the change matter.

You can compare values with our eGFR calculator, but an online calculation cannot determine the cause of a rapid decline.

Common temporary reasons eGFR falls

  • Dehydration from vomiting, diarrhea, fever, or low fluid intake
  • A recent infection or acute illness
  • Intense exercise or a high-meat meal before testing
  • Creatine supplements or changes in muscle mass
  • NSAIDs and other medicines that affect kidney blood flow or creatinine handling
  • Urinary obstruction

These possibilities do not rule out kidney disease. They explain why a clinician may repeat the blood test under stable conditions rather than interpret one result in isolation.

When a drop may represent real kidney injury

A persistent decline, reduced urine output, swelling, shortness of breath, blood in the urine, severe flank pain, confusion, or rapidly worsening laboratory values need medical evaluation. Acute kidney injury can develop over hours or days and is different from slowly progressive CKD.

What should you ask your clinician?

Ask how much the eGFR changed, whether creatinine was measured by the same laboratory method, whether UACR or urinalysis is needed, and whether any medicine or supplement could be contributing. Bring a complete medication and supplement list. Do not stop prescribed treatment without advice.

How to track the trend

Record the test date, creatinine, eGFR, illness symptoms, hydration status, and medication changes. Comparing several results is more useful than comparing two numbers without context. The CKD stage calculator can help you understand the category while you discuss the trend with a healthcare professional.

This article is educational information, not a diagnosis. Seek urgent care for severe symptoms, very low urine output, chest pain, or shortness of breath.
