Who should use the 4-variable KFRE?

The equation was developed for adults with established chronic kidney disease and an eGFR below 60 mL/min/1.73 m². It combines age, sex, eGFR, and urine albumin-to-creatinine ratio (uACR) to estimate the risk of treated kidney failure.

How to interpret the result

The result is a population-based estimate, not a diagnosis and not a guarantee of what will happen to one person. A nephrology team should interpret it alongside repeated laboratory results, symptoms, comorbidities, treatment goals, and the cause of CKD.

Why uACR matters

uACR measures albumin in urine and adds information about kidney damage that eGFR alone cannot provide. Use a recent laboratory result and ask your clinician whether it needs to be repeated to confirm persistence.
